Cook County Health’s goal in replacing its existing clinic spaces is to provide outpatient health centers that continue to support specific patient populations. For Cook County Health, this means providing healthcare for medically under-served populations and ensuring that each patient’s health journey is suited to meet their needs and removes obstacles to accessing health care.

The lack of multiple-provider services has been the primary obstacle for CCH and the need for a “one-stop-shop” model has been identified as a need by patients. The solution has been to consolidate all services under one roof. This deviates from the traditional provider care model of having services provided across multiple sites or outsourced.

The Belmont Cragin Health Center is designed in accordance with this “one-stop-shop” model. With 30 exam rooms, the Clinic provides pediatric + adult primary care, women’s health, dental care, physicals + wellness checks, cardiology + endocrinology, immunizations, diabetes management, chronic disease management, and behavioral health. The Women, Infants and Children (WIC) food assistance program is also located on site.

Creating a welcoming community resource that is transparent and accessible to all users was key to the design of the project. Construction included a steel frame with a glass exterior shell, a new parking lot, sidewalks, and new landscaping.
